Mr.Naoki Aiba

 

A ceramist and professional photographer, Mr. Aiba graduated from the Dental Technology Program at the Dental School of Aichi Gakuin University in Nagoya, Japan, in 1982. He completed the post-graduate ceramics course at the Tokai Dental Technicians School in 1986. He received the “Young Speaker of the Year Award” (presented by Dr. John McLean, OBE, of the International Society for Dental Ceramics) in 1989. In 1992, Mr. Willi Geller selected Mr. Aiba to be a member of Oral Design. He has lectured, conducted hands-on courses, and published in more than 30 countries on ceramics, dental photography, and dentist-laboratory communications. He also has published and exhibited his photographs around the world. Mr. Aiba currently serves as a member of the editorial board for QDT, and as a technical advisory board member of the Monterey-Bay Salinas Study Club (a division of the Seattle Study Club). He maintains his laboratory, Science Art, Inc., in Monterey, California, and offers hands-on seminars at his teaching facility, Oral Design Monterey.

DENTSCAPETM: Dental Photography for Dentist Laboratory Communication

Dental photography provides a means for bridging the gap between the patient and the technician. This lecture will discuss the method of overcoming three major esthetic challenges faced by the dental technician. They are shade matching, midline orientation, and incisal edge position. An award-winning professional photographer and ceramist, Naoki Aiba, CDT will explain how he overcomes those challenges utilizing digital photography.
In the lecture, he explains the fundamental of dental photography and its practical uses in dentist-laboratory communication by illustrating numbers of clinical cases fabricated solely with photographic information without seeing the patients.

臨床写真を使った歯科医師と歯科技工士間のコミュニケーション法
 「患者を見ずに、どこまで作れるか、どうやって作るか」は、私の技工士生涯を通してのテーマである。本来ならば、ケースを担当する歯科技工士が患者に接見することが最適であるが、私のように多くの歯科技工士は患者に接見することなく補綴物を作っているのが現実であろう。
 審美的補綴物製作時における歯科医師と歯科技工士の間で審美要素のコミュニケーションをする時に起こる問題として、主に以下の3点が挙げられる。
1。 歯列正中線と、顔面正中線との調和
2。 前歯切縁平面と顔面正中線、前歯切縁ラインとスマイルラインとの調和
3。 残存歯とのシェードのマッチング
 上記の問題に対応するためには、歯科医師との綿密なコミュニケーションだけではなく、独自の写真撮影と組み合わせてゴールを具現することが必要だと判った。本講ではDental Photographyの基本を説明し、審美的補綴物の製作時においていつも活用している臨床的写真の撮影法とそのプロトコル、写真とスタディーキャストを活用したコミュニケーション法を紹介し、これら方法により、患者に接見せずに補綴物を製作したケースを考察し、例証する。

Dr. Giuseppe Allais

 

Born in 1960, take the Dental Technician Diplome in the 1978, than the Doctor degree in Dentistry in 1984 at the University of Turin - Italy.
In the Years 1984-89 work as assistant professor in the Prosthetic Department of University of Turin, than between 1989 and 1993 in the Prosthetic Department and Masticatory Disorders of the University of Zurich -- Switzerland - directed from Prof. S. Palla. Fellows of Department of Oral surgery in University of Bern - Switzerland.
From 1994 conduce as, general dentist, a private practice in Turin Italy.

Additional Veneers as spirit of the time

In all industrialized country we have a decrease of tooth loss' incidence; at the same time the demand of restorative dentistry for aesthetic reasons is exponential increased.
All aesthetic material required the same or even more sacrifice of tooth structure than the traditional PFM.
In clinical situations were teeth are intact or only little compromising the aesthetic improvement has unjustified biological costs (that will affect the Patient for all life)
Additional veneers are preparation less reconstructive ceramic laminate that can improve the Patient' aesthetic maintaining intact the structure of the teeth.
In this clinically oriented presentation the authors presents the technical and clinical way to the construction of additional' veneers and solve a clinical situation.

無形成によるポーセレンラミネートベニア(アディショナルベニア)と時間との関係
 今日、先進国では歯の喪失率の減少とともに、審美的改善を目的とした修復治療が飛躍的に増加している。
 しかしながら、すべての審美的修復材料はメタルセラミックスと同等かそれ以上の歯の削合量を必要とする。また、状況により歯を削れない、もしくは少量しか削れない場合には審美的改善は不十分となり、場合によっては後に生態的な悪影響を及ぼすこともある。
 一方、アディショナルベニアは無形成で修復可能なセラミックスで、歯を傷つけることなく患者の審美的要求を満たすこともできる。
 今回の臨床に基づいたプレゼンテーションでは、アディショナルベニア製作から診療における提案などをについて述べたいと思う。

Dr. Markus B. Blatz

 

Dr. Markus B. Blatz is Professor of Restorative Dentistry and Chairman of the Department of Preventive and Restorative Sciences at University of Pennsylvania School of Dental Medicine in Philadelphia, Pennsylvania. Dr. Blatz graduated from the Albert-Ludwigs University in Freiburg, Germany, and was awarded an additional Doctorate Degree as well as Postgraduate Certificate in Prosthodontics from the same University. He is the recipient of multiple teaching and research awards and has published over 130 scientific publications, abstracts, and book chapters.
Dr. Blatz serves on the editorial boards of numerous recognized dental journals, most notably Journal of Adhesive Dentistry, Journal of Prosthetic Dentistry, Journal of Oral and Maxillofacial Surgery, Journal of Esthetic and Restorative Dentists, and was Associate Editor of Quintessence International and Quintessence of Dental Technology.
Dr. Blatz is a Diplomate of the German Society of Prosthodontics and Material Sciences (DGZPW) and a member of multiple other professional organizations, including the European Academy of Esthetic Dentistry, the American College of Prosthodontics, and O.K.U. Honor Dental Society.

Dr.Donald Maxwell Brunette

 

Donald Maxwell Brunette, PhD (Medical Biophysics) is a Professor in the Department of Oral Biological and Medical Sciences, and formerly Associate Dean Research and Graduate Studies in the Faculty of Dentistry, University of British Columbia. A founding member of the Medical Research Council (Canada) Group in Periodontal Physiology directed by Dr. A.H. Melcher at the University of Toronto, Brunette has published over one hundred articles in refereed journals and has won awards from the International Association of Dental Research, Association of Canadian Faculties of Dentistry (W.W. Wood Teaching Award) and the American Medical Writers Association (for the first edition of Critical Thinking). His research has encompassed several fields including citation analysis and clinical studies of breath odor, but is mainly concerned with cell-biomaterial interactions. He serves as a consultant to three biomaterial related journals as well as to the American Dental Association’s Council on Scientific Affairs and has been a member or chair, or scientific officer of grant evaluation committees of the Canadian Institutes of Health Research as well as the National Science Foundation (USA).

Dr. Vittorio Cacciafesta

 

Dr. Vittorio Cacciafesta received his dental training at the University of Naples “Federico II”, Italy. From July 1995 until August 1996 he was Research Fellow at the Department of Orthodontics, Humboldt-University of Berlin, Germany. From September 1996 until August 1999 he was Orthodontic Resident at the Department of Orthodontics, Royal Dental College, Aarhus University, Denmark, where he received the Specialty and Master of Science in Orthodontics. He is “Dottore di Ricerca” (Research Doctorate) in Biotechnology of Dental Materials. Since 1999 he is Assistant Clinical Professor at the Department of Orthodontics, University of Pavia, Italy. From 1999 until 2002 he was Visiting Professor at the Department of Orthodontics, Aarhus University, Denmark, and Visiting Professor at Temple University, Philadelphia, USA. He has published over 100 refereed publications and 4 book chapters. He is Co-Editor of “Ortodonzia Clinica”, Associate Editor of “Progress in Orthodontics”, and Referee for the following scientific journals: “American Journal of Orthodontics and Dentofacial Orthopedics”, “Angle Orthodontist”, “European Journal of Orthodontics”, “Journal of Clinical Orthodontics”, “Orthodontics and Craniofacial Research”, “Ortognatodonzia Italiana”. His main research interests are bonding materials, brackets, metallurgy, friction between brackets and wires. His main clinical interests are biomechanics, lingual orthodontics, orthodontic treatment of adults. He has been lecturing extensively in Europe, Middle East, Asia and USA.

New horizons in adult orthodontics: where is the limit?

In recent years, there has been a dramatic increase in the number of adults receiving comprehensive orthodontic treatment. In clinical practice, management of adults may be somewhat different than for most adolescents. Adults are more likely than adolescents to have dentitions that have undergone some degree of mutilation over time, which may necessitate alterations in the treatment strategy. Excessive wear, missing and compromised teeth are some of the differences observed. For adult patients who have experienced bone and attachment loss as a consequence of periodontal disease, a new set of biomechanical requirements exists for producing the tooth movements desired during the orthodontic therapy. Proper planning and appliance design for adults presenting with multiple missing teeth may require some degree of creativity to adapt to unique malocclusions and deliver the proper biomechanical force system. In this presentation, current clinical application of biomechanical principles will be presented with emphasis on clinical orthodontic management of severely mutilated dentitions.

成人矯正の新たな広がり:その限界とは?
 ここ数年、包括的な矯正治療を受ける成人の数が劇的に増加している。臨床上、成人の治療方法は、一般的な成長期の患者と比べて若干異なる。成人は成長期の患者に比べ、より多くの欠損した歯列をもっていることが多いため、しばしば治療計画の変更を必要とする場合がある。両者の違いとしては、成人には極端な咬耗や歯牙の欠損、継続歯などが多くみられる。また長年の歯周疾患により、歯槽骨や付着歯肉が吸収および退縮した成人の患者さんに対して、矯正歯科治療の期間中、安定した歯牙移動が可能なように新しいバイオメカニカルな条件を備えた便利な材料がある。成人のための適切な治療計画と装置のデザインについては、たとえば多数歯が欠損しているような症例では、特殊な不正咬合に適合し、適切なバイオメカニカル・フォース・システムを伝えられるように、ある程度の創造力を必要とすることもある。今回の発表では、バイオメカニカルな原理を臨床応用した最新例として、極端に崩壊した歯列を矯正で治療する方法を特別にご紹介する。

Dr. Gunner E. Carlsson

 

Dr. Carlsson was Professor and chairman of the Department of Stomatognathic Physiology (with its clinic for TMD patients) at the Faculty of Odontology, University of Gothenburg, Sweden, 1969 to 1987, and Professor and chairman of the Department of Prosthetic Dentistry at the same faculty 1987 to 1995. He is now Professor emeritus. He was Dean of the Faculty of Odontology 1974-1977 and 1987-1990.
Dr. Carlsson has published about 400 articles and several book chapters on prosthodontics, occlusion and temporomandibular disorders. He was editor-in-chief of the International Journal of Prosthodontics 1997 to 2004 and held the same position for the Acta Odontologica Scandinavica 1991 to 1997. He was Co-President of the International College of Prosthodontists 1998-1999. He has continued his scientific activity and international lecturing, and he has been author or co-author of > 30 publications during the last 5 years (2005-2009).

What is new regarding etiology, diagnosis and treatment of TMD?

The topic of temporomandibular disorders (TMD) has for long been controversial.
Are occlusal disturbances still considered an important etiological factor for TMD? How are bruxism and other oral parafunctions related to TMD? Do we need sophisticated instruments in diagnosing TMD? What is the role of occlusal therapy and oral splints in management of TMD? The aim of the presentation is to discuss current opinions on TMD and clinical diagnosis and management of specific diseases of the TM joints. Recent research has paid attention to the co-morbidity of TMD and a number of other disorders. How to interpret this relationship?

顎関節症の病因、診断、治療に関する最新の知見
 顎関節症(TMD)の話題は永く議論の対象である。いまだに咬合の障害がTMDの主要な原因と考えられているのだろうか? また、ブラキシズムやTMDに関係する他の異常機能(パラファンクション)はどうだろう。TMDの診断に高性能な最新機器が必要だろうか。TMDの治療における咬合治療やスプリントの役割とは何だろう。今回の講演では、最近のTMDに関する見解、また顎関節のいくつかの病変の臨床診断および管理に関して考察するのが目的である。最近の研究は、TMDとその他の多くの障害との共存に注目している。これらの関係をいかに読み解けばよいのだろうか?

Excellence and Rational Approach in Post-Endodontic Composite Restoration

At present day tooth preparation, using correctly adhesive materials and techniques, and performing tooth-colored restorations are the major objectives of modern restorative dentistry also in post-endodontic restoration.
Rebuilding an endodontically treated tooth usually requires an immediate coronal seal to prevent bacterial microleakage and consequently the failure of endodontic treatment. We can preserve a great amount of sound enamel and dentin if we restore anterior and posterior tooth with direct composite. We need to minimize tooth preparation and to achieve a proper indication and technique to use the post or realize a core built up. It must be considered the great opportunity of indirect adhesive restoration to delay the full crown.

歯内療法後のコンポジット修復の優位性と合理的アプローチ
 今日、適正な接着材料とその技術を用い歯の色調調整修復を行うことは根管治療後の修復においても主要な目的の一つである。歯内療法後の歯牙の再建では、細菌の微少漏洩とそれに続く根管治療の失敗を防ぐため速やかに歯冠部を封鎖しなければならない。 直接法でコンポジット修復すれば前歯臼歯ともに健全なエナメル質と象牙質の多くを温存できる。形成量を出来るだけ少なくしポストやコア作成の適応を選び最適なテクニックを用いなければならない。そうすれば、間接接着修復は全部冠への移行を遅らせる有用な手法と言える。

Perio-orthodontic Relationship

Dr. Dietschi Didier

 

Dr Didier Dietschi was licensed in 1984 and got his doctoral degree in 1988, at the University of Geneva, Switzerland. After a 6-year period of full time teaching and research activity in Operative Dentistry and Periodontology, he maintains a part time activity in a private office in Geneva, dedicated to aesthetic restorative dentistry. Dr. Dietschi has published more than 45 clinical and scientific papers on adhesive and aesthetic restorations; he also co-authored the book “Adhesive Metal-free Restorations”, edited in 1987, by Quintessence. Dr. Dietschi is lecturing internationally on adhesive & aesthetic restorations.

The use of composite in simple to complex functional and aesthetic rehabilitations: “a lot better than what we might think!”

Due to the tremendous progresses made in their optical properties, wear resistance and application technique, modern composite resins offer conservative, biological and aesthetic solutions to many problems, which formerly could only be approached by expensive and more invasive ceramic restorations. In addition, composite resins allow the aesthetic concerns of many patients to be fulfilled, without creating additional damage to remaining, healthy tissues. Recently, composite resins have even received attention for the treatment of complex abrasion/erosion cases. Practically, their only remaining drawback is practitioner experience and knowledge of their broad potential in restorative dentistry.
This program will cover treatment rationale and integration of direct composites in a comprehensive approach to dental aesthetics and occluso-functional rehabilitations; a detailed description of clinical procedures will complete the program and provide attendees with a full topic picture.

臨床におけるコンポジットレジンの活用法
―シンプルなケースから機能的、審美的問題を抱えた難症例まで:その想像以上の優位性―

 コンポジットレジンの光学的特性、耐摩耗性、接着技術は凄まじく進歩した。そのおかげで、今日のコンポジットは多くの症例に対して、歯質の温存を可能とし、生物学的、かつ審美的治療方法を提供できるようになった。これらの症例に対して、以前は高価であり歯質削除量の多いセラミックによってのみ対応してきたが、コンポジットレジンであれば残された健全歯質を削除することなく、多くの患者の審美的要求を満たすことができる。さらに近年は、摩耗や浸食といった症例の治療法としても注目されはじめている。しかしながら、実際、本修復法の問題として依然と残るのは、臨床医にとって多くの経験と保存修復学の幅広い知識を必要とすることである。
 本講演では、審美的、そして咬合面の機能的回復におけるダイレクトコンポジットの包括的アプローチにおける原理、そして審美と機能の統合について述べる。最後に、関心の高い症例写真を用いて、その臨床的手順の詳細について説明する。

Dr. Eric Van Dooren

 

Dr. Eric Van Dooren attended the Katholieke Universiteit Leuven, Belgium, where he received his degree in dentistry in 1982. After graduating he opened a private practice in Antwerp, Belgium, which is limited to periodontics, fixed prosthodontics, and implants.
Currently Dr. Van Dooren is a active member of the European Academy of Esthetic Dentistry. In addition, he is a member of the editorial staff of Teamwork (Germany) and a member of the Belgian Dental Society.
Dr Van Dooren lectures nationally and internationally , mainly on aesthetics,implants and aesthetic periodontal surgery.

Conquering the Challenge of Minor and Major Defects in the Aesthetic Zone: Improving the Results with Teamwork

Minor and major defects in the aesthetic zone often confront dentists with the limitations of the periodontal and prosthetic treatment options. It is important to understand these limitations. Proper diagnosis, analysis and treatment planning will minimize failures, and teamwork is essential in this part of the therapy. This lecture will focus on the dentist’s communication with the lab technician and the patient in order to maximize the final result. Once the treatment plan is established it is important to minimize treatment time and trauma. Choosing the proper prosthetic components in restorations is crucial to achieving state-of-the-art aesthetic and functional results.
Upon completion of the presentation, attendees should be able to:

Daniel Edelhoff granted his Certified Dental Technician degree from the Dental technician school in Dusseldorf. He graduated from Dental School in 1991 and earned his Doctor of Medical Dentistry degree from the University of Aachen in 1994. He served as Visiting Professor at the Dental Clinical Research Center of the Oregon Health and Sciences University, Portland, Oregon from 1999 to 2001. He granted his Ph.D. in June 2003 at the University of Aachen, Germany and is currently Tenured Associate Professor at the Department of Prosthodontics and Dental Material Sciences at the Ludwig-Maximilians-University in Munich. Daniel Edelhoff is board certified Specialist in Prosthodontics and holds memberships in the German Society of Oral and Maxillofacial Medicine and the German Society Dental Prosthetics and Material Sciences (DGZPW). He is member of the managing committee of the German Association of Oral Implantology (DGI) and the advisory board of the DGZPW. Daniel Edelhoff received an international research grant of the German Scientific Society (DFG) from 1999 to 2001. He is the author of numerous clinical and research articles as well as scientific statements.

New Approaches in the Restoration of Complex Cases Including CAD/CAM-Technology

Highly predictable results in the restoration of complex cases necessitate the approach of clinical and technical aspects in teamwork between dentist and dental technician. Templates generated from the diagnostic wax-up and provisional restorations play essential roles in the communication between patient and the dental restorative team. Since CAD/CAM-technology has accessed innovative restorative materials like high-density polymers, new treatment options are available for long-term provisional restorations as a functional and esthetical test drive of the final restoration. The lecture will present new restorative approaches following a strict protocol in team communication to economize tooth structure removal according to material requirements and to reduce the risks of failures by an extended provisional.

CAD/CAM技術を応用し複雑な症例に対する新しい修復方法
 複雑な症例で予知性の高い結果を得るためには、歯科医師と歯科技工士のチームワークのもと、チェアサイド・ラボサイドでの検討が必須である。診断用ワックスアップとプロビジョナルレストレーションから得られたサージカルテンプレートは、患者と歯科の修復チーム間のコミュニケーションに重要な枠割を果たす。また、歯科用CAD/CAM技術により、高密度ポリマーのような新しい修復材料が加工可能になった。これにより、最終修復物の機能面・審美面でのテストドライブとして、長期的にプロビジョナルレストレーションを使用するという新しい治療方法が可能となる。本講演では、材料特性にもとづいて歯質削除量を最小限にし、また、プロビジョナルレストレーションの期間を延長することによって、修復失敗の危険性が減少するように試みた、チームコミュニケーションにおける正確なプロトコルにしたがった新しい修復方法を解説する。

Dr. Joseph P. Fiorellini

 

Dr. Joseph Fiorellini is Professor and Chair of the Department of Periodontics at the University of Pennsylvania School of Dental Medicine. In 1990, he received his DMD from the Harvard School of Dental Medicine and in 1993 was awarded the DMSc degree in the field of Oral Biology and a Certificate in Periodontology. Dr. Fiorellini has published over 50 manuscripts, reviews or textbook chapters and received awards related to research including NIH and IADR research travel grants, The AAP Young Investigators Award, The Academy of Osseointegration Research Award, and The European Association for Osseointegration Research Prize. Currently, he serves on the editorial board of the Journal of Periodontology, the International Journal of Oral and Maxillofacial Implants and The International Journal of Periodontics and Restorative Dentistry.

The fields of guided tissue and bone regeneration have undergone a change in paradigms over the past several years. Conventional procedures to regenerate oral tissues lost to disease or trauma most often involve the use of barrier membranes. These techniques may foster selective cell repopulation and regrowth of the periodontium or osseous structures. Since the predictability of these methods may be limited to certain case types, efforts have begun to investigate the possibility of harnessing the regenerative potential of recombinant molecules. Numerous proteins have been identified that are potent biologic mediators which promote many of the events in wound healing including cellular proliferation, chemotaxis and connective tissue matrix synthesis. These effects are most apparent in the cells involved in tissue repair such as mesenchymal cells, osteoblasts and osteoclasts. Clinical results demonstrate a striking regrowth of periodontium or bone utilizing recombinant molecular mediators.

BPS – Biofunctional Prosthetic System
The treatment of edentulous patients under consideration of function, phonetic and aesthetics

Further education in the field of complete and partial dentures has not been very popular lately, particularly when comparing it to fixed prosthetics. Nevertheless, there is still a great demand for this kind of dental treatment in daily practice, and to some extent, even a growth trend is noticeable.
The successful fabrication of conventional dentures or implant-supported hybrid prostheses for edentulous patients presupposes a perfect communication between dentist and dental technician.
The aim of this presentation is to review the basic steps to determine the intermaxillary-relationship and to present an attractive clinical and technical concept including the innovative BPS System.
Beginning from the fabrication of preliminary impressions and establishing of a correct vertical and horizontal intermaxillary-relationship by using the Centric Tray which is an important and critical step during treatment of edentulous cases, until the individualised determination of an optimised jaw-relation, this presentation includes an attractive clinical concept.
Furthermore all-important technical steps according to the BPS philosophy will be presented and detailed discussed.
A clinical and technical treatment concept covering all-important aspects from first impression to incorporation of finished dentures on the basis of teamwork between dentist and dental technician is presented.

BPS:生体機能的補綴システム ―機能・発音・審美を考慮した無歯顎患者の治療法―
 近年、フルデンチャーやパーシャルデンチャーなど可撤性義歯の研修人気が、固定性補綴物に比べて薄れてきている。それにもかかわらず、可撤性義歯による歯科治療の需要は日常臨床の中でいまだ多く、かつ増加の一途をたどっていることは注目すべき点であろう。無歯顎患者において、従来型の義歯あるいはインプラントオーバーデンチャーの製作を成功に導くには、歯科医師と歯科技工士の完全なコミュニケーションが欠かせない。
 そこで今回のプレゼンテーションでは、顎間関係決定の基本的なステップを明らかにし、また革新的なBPSシステムを含む、魅力的な臨床と技工のコンセプトを提示する。まずは義歯の一次印象に始まり、セントリックトレーを用いた正確な垂直・水平顎間関係を確立することは、無歯顎症例の治療において個々の最適な顎間関係を決定する重要でかつ決め手のステップである。

Additional Veneers as spirit of the time

In all industrialized country we have a decrease of tooth loss’ incidence; at the same time the demand of restorative dentistry for aesthetic reasons is exponential increased. All aesthetic material required the same or even more sacrifice of tooth structure than the traditional PFM.
In clinical situations were teeth are intact or only little compromising the aesthetic improvement has unjustified biological costs (that will affect the Patient for all life) Additional veneers are preparation less reconstructive ceramic laminate that can improve the Patient’ aesthetic maintaining intact the structure of the teeth.
In this clinically oriented presentation the authors presents the technical and clinical way to the construction of additional’ veneers and solve a clinical situation.

無形成によるポーセレンラミネートベニア(アディショナルベニア)と時間との関係
 今日、先進国では歯の喪失率の減少とともに、審美的改善を目的とした修復治療が飛躍的に増加している。
 しかしながら、すべての審美的修復材料はメタルセラミックスと同等かそれ以上の歯の削合量を必要とする。また、状況により歯を削れない、もしくは少量しか削れない場合には審美的改善は不十分となり、場合によっては後に生態的な悪影響を及ぼすこともある。
 一方、アディショナルベニアは無形成で修復可能なセラミックスで、歯を傷つけることなく患者の審美的要求を満たすこともできる。
 今回の臨床に基づいたプレゼンテーションでは、アディショナルベニア製作から診療における提案などをについて述べたいと思う。

Dr. Ueli Grunder received his DMD degree from the University of Zurich, Switzerland, in 1982. His post-graduate education in advanced fixed prosthodontics also came from the University of Zurich, where he is senior lecturer since 1987. He is a certified specialist for reconstructive dentistry.
He maintains a private practice together with Thomas Gaberthüel since 1989 in Zollikon-Zurich and has published numerous papers and extensively lectured nationally and internationally on the surgical and prosthetic aspects of implant dentistry.
Dr. Grunder is a Past-President of the Swiss Society of Oral Implantology (SSOI) and a Past-President of the European Academy of Esthetic Dentistry (EAED).

Aesthetics in Implantology - Predictable Site Development

The most challenging field in implant dentistry is still the aesthetic zone. Appropriate bone and soft tissue engineering enhances the aesthetic performance of long-lasting restorations that blend in perfectly and emerge from the periimplant sulcus in harmony with the neighbouring teeth.
The surgical procedure can be planned based on the pre-operative situation, whereby the difficult lies in selecting among a variety of surgical methods the one that can offer the best possible esthetic result in each particular case. Upon completion of this presentation, participants should be able to:
- Understand the need for augmentation procedure in various situations
- Know the different techniques and the biological limits of augmentation procedures (bone and soft tissue)

Dr. Eric Herbranson

 

Dr. Eric Herbranson is co-founder and Executive Director of Brown & Herbranson Imaging, a company that develops dental and human anatomy education software under the eHuman moniker. He is also the developer of the Xmount series of microscope camera mounts. With over 30 years in practice, Dr. Herbranson is a dedicated clinical endodontist. He has made a significant contribution over the last 20 years as clinical assistant professor at the University of the Pacific School of Dentistry where he lectures to students and special interest groups on endodontics, technology in dentistry, and microscope photography. Dr. Herbranson’s study of physics and 40 years experience in film and digital imaging provide him with an educated understanding of macro and microphotography, and affords him a unique vision of endodontic education and image production. With his innovative approach and advanced imaging skills, Dr. Herbranson developed the unique processes and methodology for capturing images of human and dental anatomy now used as the basis for Brown & Herbranson Imaging’s educational technology.
Dr. Herbranson is the co-author of the chapter on tooth anatomy in Pathways of the Pulp, the top selling textbook on endodontics. Dr. Herbranson is a frequent speaker and educator at universities and conferences on the subjects of integration of new technology into dentistry, the use of software and computers in presentations, and surgical operation microscope photography.. Strategies for Periapical Disease; What Works?

This lecture will review current concepts and controversies in endodontic therapy and how they impact successful treatment. After review of the bacterial basis of endodontic disease it will discuss strategies for dealing with the disease they create. Three major areas will be covered. The first is the current understanding in tooth anatomy and the technology available to find and access that anatomy. The second area will be a discussion of the latest concepts in irrigation and disinfection of the root canal systems. The third will be a review and illustration of the latest concepts for restoration of endodontically treated teeth.

Contemporary endodontics: saving the natural tooth!

New imaging techniques, electronic devices for determination of endodontic working length, highly flexible NiTi instruments for the preparation of even severely curved root canals, the dental operating microscope and advanced obturation techniques and materials have enabled the clinician to perform high quality treatment with predictably high success rates. On the other hand microbiological research has clarified the nature and extent of the bacteriological problems (biofilm) the clinician is facing when treating a tooth with a necrotic pulp and apical periodontitis. To overcome these microbiological problems simple devices such as a rubber dam and a thorough concept of disinfection (irrigation and medication) are mandatory.
The lecture intends to present an actual concept for endodontic treatment based on current knowledge on microbiology and to demonstrate when and how certain instruments can be useful adjuncts in such a contemporary treatment concept.

歯を残すための近代的な根管治療
 コーンビームCT、作業長を教えてくれる電気根管長測定機、大きな湾曲根管でもスムーズに拡大可能なNi-Tiファイル、手術用マイクロスコープ、緊密な根管充填を可能にする根管充填法や材料の開発といったさまざまな技術革新により、臨床家は高い成功率で質の高い根管処置を行えるようになった。その一方で、微生物学の研究は、臨床家が歯髄壊死や根尖性歯周炎を治療するときに直面する細菌学的問題(バイオフィルム)の本質や困難さを明らかにしている。この細菌学的な問題を克服するためには、ラバーダムの使用や根管の完全な無菌化(洗浄、抗菌剤の投与)といった単純かつ基本的な処置が必須である。
 この講演では、最新の微生物学に則った根管処置のあり方と、それを実践するために、いつどんな器具を用いればよいかについて提示を行う予定である。

COMPOSITE ARTISTRY - SIMPLIFIED

The greatest impact of cosmetic or esthetic dentistry is providing patients with beautiful smiles that enhance self-esteem and encourages referrals. However, materials and techniques have undergone significant evolution in recent years and dentists are challenged to keep current. In addition, esthetic standards have been raised and quality outcomes redefined by today’s educated patient. This presentation will illustrate the use of Empress Direct (Ivoclar), a new naturally shaded composite system, for treating anterior restorative needs and elective cosmetic enhancements. The logical, conservative techniques shown will lead to results that please the patient and the dentist.

Implant position and subgingival contour for harmoneous Gingival line in Esthetic zone

The dental treatment spectrum has been changed and improved dramatically since implantology has been introduced in dentistry last few decades. There is no doubt about osseointegration between implant fixture and alveolar bones. Even with the help of GBR and other wonderful procedures, patients with compromised dental conditions can be treated with implant treatments. However, patients expectations on dental treatments are rising higher. Patients are more interested in not only functions but also aesthetic and comforts.
In order to achieve aesthetic results it is necessary to analyze tooth morphology and gingival morphotye to establish natural looking gingival line. Every patient has different conditions to start with and there exist some requirements to be applied to each patients. For esthetic implant treatment result, three dimensional position of fixture placement and super structure form from the fixture should be well understood and evaluated.
In this context, the lecture describes some necessary conditions for constructing aesthetic and physiologic super-structures, abutment and final prosthesis, in implant treatments. Decision making criteria on fixture placement based on vertical position, mesio-distal and bucco-lingual angle will be covered as well as proper subgingival contouring management when constructing supra structure of implant prosthesis based on tooth morphology and gingival morphotype. Aesthetic implant treatment results following patients' own gingival line can be achieved when those conditions are satisfied.

 この数10年間の歯科へのインプラント学の導入以来、歯の治療スペクトルは、変化し、劇的に進歩してきた。インプラントフィクスチャーと歯槽骨間のosseointegrationには疑いはない。 GBRや他の素晴らしい手順のおかげで、妥協の必要な口腔内状況の患者にもインプラント治療は可能である。 しかしながら、患者の歯科治療に対する期待は、より上昇している。 患者は機能だけではなく、審美性と快適性にもさらに興味を持っている。
 審美的な結果を獲得するためには、自然観のある歯肉ラインを確立するための歯の形態と歯肉の形態型の分析が必要である。 すべての患者は、初診時に異なった状態であり、個々の患者に適用されるべき要件も異なる。したがって,審美的なインプラント治療のためには、フィクスチャーの三次元埋入位置と上部構造のフィクスチャーからの移行形態をよく理解して評価することが必要である。
 そこで、本講演では、インプラント治療における審美的で生理的な上部構造・アバットメン・最終補綴装置の構築のために必要ないくつかの条件を述べる。垂直的位置と近遠心・頬舌的傾斜に基づいたフィクスチャー埋入の原則や、歯の形態と歯肉の形態型に基づいたインプラント上部構造作製時の歯肉縁下カントゥアの適切なマネジメントについても述べる。これらの条件が満たされた場合に、患者自身の歯肉ラインに調和した審美的なインプラント治療結果が得られるのである。

Ronald Jung is trained in oral surgery, prosthodontics and implant therapy. He is currently Vice Chairman of the Department of Fixed & Removable Prosthodontics and Dental Material Sciences at the University of Zurich in Switzerland (Chairman: Prof. Dr. Ch Hämmerle). In 2006 he worked as Visiting Associate Professor at the Department of Periodontics at the University of Texas Heath Science Center at San Antonio, USA (Chairman: Prof. Dr. D. Cochran).
He is an accomplished and internationally renowned lecturer and researcher, best known for his work in the field of hard and soft tissue management in implant dentistry and his research on new technologies for tissue regeneration.

Interdisciplinary approach in aesthetic implant therapy:
from material selection to clinical success

Titanium is clinically successful and well documented in implant therapy. The increasing demand of metal-free therapeutic solutions advances the development of all-ceramic reconstruction. The material performance and the clinical success of all-ceramic reconstructions will be discussed.
The decision of using either metal or all-ceramic abutments is based on criteria including material science, clinical performance and aesthetics. Clinical and experimental data will provide information on the color-change effect of all-ceramic restorations, based on all-ceramic abutments compared with PFM restoration based on titanium abutments on marginal peri-implant soft tissue. Decision criteria will be formulated in order to chose between all-ceramic abutment and crowns as well as titanium abutments and porcelain fused to metal crowns.

審美インプラント治療におけるインターディシプリナリーアプローチ
―その材料選択から臨床的成果まで―

 インプラント治療において、チタン材料は臨床的な成果をあげており、論文による裏付けも十分にある。また、メタルフリーの治療に対する需要が高まるにつれ、セラミックスによる補綴は進歩してきた。本講演では、これらの材料と臨床におけるオールセラミック修復の成功について考察する。
 アバットメントの選択をメタルにするかオールセラミックにするかは、材料特性、臨床成績や審美性といった基準に基づいて判断される。そこでまず、インプラント周囲の軟組織辺縁において、チタン製アバットメントに装着された陶材焼付け鋳造冠と比較して、オールセラミックアバットメントを使用したオールセラミック補綴がどのように色調に変化があるかについて、臨床データと実験データを用いて提示する。さらに、オールセラミックアバットメントとクラウンなのか、それともチタン製アバットメントと陶材焼付け冠を選択するのかの判断基準についても解説してみたい。

Dr. Kan completed his training in Prosthodontics and a Master degree from the Implant Dentistry from Loma Linda University School of Dentistry (LLUSD) in 1997. He is currently a Professor in LLUSD and maintains a private practice limited to Prosthodontics and Implant Surgery.
Dr. Kan was the recipient of the Best Research Award from the Academy of Osseointegration in 1997. He received the Judson C. Hinckey Award from the Journal of Prosthetic Dentistry in 2003 and the Robert James Achievement Award for his contribution in Implant Dentistry in 2005.
Dr. Kan is on the Periodontology/Implantology Editorial Board of the Practical Periodontics & Aesthetic Dentistry. Besides lecturing both nationally and internationally, he has published over 40 reference articles and chapters in textbooks.

Immediate versus Delayed Implant Placement in the Esthetic Zone: Science, Art & Limitations

Achieving anterior implant esthetics is a challenging procedure. To create implant restorations with harmonious gingival contour that emulate nature is a fusion of science and art. Understanding the biologic and physiologic limitations of the soft and hard tissue will facilitate predictability in simple to complex esthetic situations.
This presentation will focus on current implant treatment philosophies and methodologies on immediate versus delayed implant placement in the esthetic zone. Equal emphasis will be placed on the diagnosis and treatment planning, surgical, prosthetic and complications management of soft and hard tissue for optimal anterior implant esthetics.

審美領域におけるインプラントの即時埋入と遅延埋入:科学、技術、限界について
 前歯部におけるインプラント審美の達成には難易度の高い術式が必要であり、また、自然の状態に近づけ、調和した歯肉カントゥアを有するインプラント修復物の製作には科学と技術の融合が必要である。軟組織および硬組織における生物学的および生理学的な限界を理解することは、シンプルな症例から複雑な症例に対する予知性を知るうえでの手助けとなるであろう。

The Role of the Dental Hygienist in Treatment of Peri-implant diseases

Recent studies seem to confirm that peri-implant infection may represent the most frequent implant treatment complication in the long run. This condition may lead to the loss of the implant with all related consequences easy to foresee. The presentation will first focus on early diagnosis and factors related to peri-implantitis and relative preventive measures. Treatment of initial peri-implant infection will be illustrated by clinical cases and basic principles of non-surgical approach addressed. Clinical evidence is increasingly demonstrating that a strict maintenance is crucial to obtain healthy peri-implant tissues, and therefore instructions to the patients have to be precise and thorough.

インプラント周囲疾患における歯科衛生士の役割
 近年の研究では、インプラント治療後の長期経過におけるほとんどの問題はインプラント周囲軟組織の炎症であり、かなり頻繁に生じることが示されています。このインプラント周囲軟組織の炎症が、やがてインプラントの喪失を引き起こすことは容易に察知できます。また、徹底的なメインテナンスによって、インプラント周囲軟組織の健康を獲得できるという臨床報告が増加しています。だからこそ、患者さんへの指導は正確で綿密であるべきです。
 今回の講演では、まず初期の兆候の診断とインプラント周囲炎に関連する事実に焦点を当て、次に相対的な予防を述べます。さらに、初期のインプラント周囲の感染について、症例を通して基本となる非外科的アプローチを紹介します。

Implant treatment in the anterior maxilla: planning considerations, surgical concepts and changing techniques for esthetic success

Conventional concepts in esthetic dentistry generally focus on establishing ideal tooth form and color, but often overlooked is the challenging interplay with the surrounding gingival framework. This lecture will focus on the esthetic outcome of implant treatment. An understanding of how to control and enhance tissue quality and contour is a crucial element to a highly successful esthetic result. Current bone and soft tissue grafting techniques, new implant and abutment designs and changing surgical and restorative treatment concepts that contribute to the long-term esthetic success of restorations supported by single and multiple implants will be presented

Management of teeth with deep angular bone lesions Regeneration / extraction /implants

The pathogenesis of that variety of periodontitis that results in the formation of angular bone lesions will be discussed. Issues including (i) the spread of the infiltrate, (ii) the involvement of bone tissue, (iii) the effect of “trauma from occlusion” will be deliberated. The importance of establishing a correct diagnosis of the case and the lesion prior to the onset of treatment will be emphasized. Sites with plaque associated lesions and lesions of endodontic origin will be presented and treatment options discussed.
The outcome of treatment of intrabony lesions at single teeth including the use of barrier membranes and biomaterials (including single implants) will be presented.

深い骨欠損への対処法:骨再生療法/抜歯/インプラント
 本講演では、深い骨欠損を形成するさまざまな歯周疾患の発生機序について論ずる。そして、(1)感染の拡大 (2)骨組織の障害 (3)咬合性外傷の影響 といった問題について考察してみたい。症例に対して適切な診断を確立することと、治療開始前の病巣はとても重要であることをとくに示したい。病巣に関係するプラークが停滞している部位、エンド由来の病巣についても治療法のオプションも含めて提示する。バリアーメンブレンとバイオマテリアル(また単独歯インプラント)を用いた単独歯の骨縁下欠損に対する治療結果についても提示する。

Indirect techniques for conservative aesthetic restorations: evidence based approach for a predictable result

A beautiful smile is the first wish of our patients: this result can be often achieved with materials and clinical procedures that save the tooth structure and require a low biologic sacrifice.
Nowadays, there is a huge employment of indirect techniques in aesthetic restorative dentistry, because the new adhesive technologies, together with the evolution in the aesthetic materials (resin composites and ceramics) allow obtaining predictable and easily controllable results over time. This can be achieved with a strict observance of the operative protocols.
This speech will define the guidelines to be followed, analyzing the state-of-the-art of the techniques and materials available to reach a good result in indirect restorative dentistry.

Balancing the critical inputs to successful treatment in implantology: cooperation between the clinician, technician, and technology

Esthetic implant restorations are still challenging procedures, despite the rapid advances in treatment methods and the expanded options that they provide. A successful approach to esthetic treatment involves a high degree of systematic diagnosis, planning and implementation. This goes hand-in-hand with ongoing training and education for the whole clinical team to ensure consistency and predictability. In particular, it is essential that the clinician and the technician work closely to define a clear path to achieving the optimal smile for each of our patients.
Among others, the lecture will focus on new techniques to optimize the implant restoration’s emergence profile and achieve predictable, natural esthetic results.

Bisphosphonate Induced Osteonecrosis of the Jaws (BIONJ)

Both oral and intravenous nitrogen containing bisphosphonates cause Bisphosphonate Induced Osteonecrosis of the Jaws (BIONJ). Intravenous BIONJ cases are more severe and less amenable to office based debridement surgeries. Preventive dentistry measures that avoid extractions, dental implant surgery, and periodontal surgery can prevent many cases. In cases of established intravenous BIONJ palliative management with antibiotics and 0.12% chlorhexidine can be used versus continuity resections for cure.
Oral BIONJ cases are mostly due to Alendronate. The same prevention approach applies as with intravenous bisphosphonates. However, treatment for resolution can be accomplished with a drug holiday guided by serum CTX testing and local debridement surgery in most cases.

ビスフォスフォネート誘発性顎骨壊死
 窒素含有のBP製剤は、経口薬でも静注薬でも誘発性顎骨壊死(BIONJ)の原因となる。静注薬により発生したBIONJはより重篤で、外来でのデブライドメントはあまり効果がない。歯科的な予防策は、抜歯、インプラント、歯周外科などを避けることである。静注薬により発生したBIONJに対する対症療法は、切除を繰り返し行う方法だけでなく、抗菌薬や0.12%クロルヘキシジンの利用も用いられる。
 経口薬で発生したBIONJはほとんどがアレンドロン酸によるものである。予防策はBP製剤の静注薬投与が行われている場合と同じである。しかしながら、ほぼすべての症例で、服薬を中止し、血清中のCTX (type 1 collagen) を計測し、局所のデブライドメントを行うことで完治している。

Lasers in Endodontics, Cavity Preparation and the Treatment of Hypersensitive Dental Necks

Among many other indications for the use of lasers in dentistry, there are three fields of application, where the laser has brought major proceedings and specific advantages: Endodontic treatment, cavity preparation and the treatment of hypersensitive dental necks. Different lasers are being used in root canal preparation, disinfection of canals and surrounding dentinal tubules. Thus laser treatment will significantly ensure the long term success of endodontic treatment. Other wavelengths are used in the fields of cavity preparation: The Erbium laser group. Enamel and dentin can be ablated without thermal side effects due to the photo ablative impact of the wavelengths. Composite restorations can be placed without acid etching because of the retentive surface delivered by the laser. The CO2 laser (10600nm) is widely used in surgery producing very satisfying results. Another application for this wavelength in conservative dentistry is the treatment of hypersensitive dental necks. In most cases one appointment is sufficient to achieve permanent freedom from pain even in patients who suffer from severe symptoms.

歯内療法、窩洞形成、歯頸部知覚過敏におけるレーザー治療
 歯科治療におけるレーザー使用について多く示唆されるなか、広く応用され、はっきりとした有効性がある3つの分野がある。すなわち、歯内療法、窩洞形成、そして歯頸部知覚過敏におけるレーザー治療である。
 根管拡大、感染根管処置、そして側枝に対しては異なるレーザーが使用されている。このようなレーザー治療は歯内療法処置の長期予後を格段に良くしてくれるであろう。窩洞形成には、また異なった波長のレーザーが使用される。Er:YAGレーザーである。レーザー光による蒸散作用により熱によるエナメル質、象牙質にダメージを与えることなく形成ができる。コンポジットレジン修復においては、レーザーにより維持力のある歯質を得ることができるため、酸エッチング処理を行わずに充填処置を行うことができる。
 炭酸ガスレーザー(10,600nm)は、外科処置において満足する結果を得ることができるとして広く応用されている。この波長のレーザーのもうひとつの応用として歯頸部知覚過敏がある。たとえ強い症状を訴える患者でも、ほとんどの症例において、一度の処置で痛みを取り除くことができる。

Cosidering Factors for Maxillary Posterior Implantation with Sinus Augmentation

Implant has become a very common therapy within dental area.
However, annual increase in trouble cases brings demand for secure implant therapy. Especially, implantation of maxillary posterior area can be challenging because of various reasons including atrophy of remaining bones. In many cases, it requires sinus augmentation as well. If sinus augmentation fails, it can bring complication such as maxillary sinusitis and existing bone destruction from inflammation. Both complications often cause patients to suffer from aftereffects. Accordingly, discussing the guideline for sinus augmentation is important.
Currently, two methods are used in Maxillary sinus floor elevation which are Lateral approach method and Internal approach method. There are various divergent opinions about guidelines for different situations because of advantages and disadvantages of both methods.
In this presentation, I would like to talk about considerations in performing Maxillary posterior implantation and my efforts for secure sinus augmentation.

 インプラントは歯科領域において非常によく用いられる治療である。しかしながら年々増加するトラブルから、安全なインプラント治療が求められている。特に上顎臼歯部におけるインプラント埋入は、残存骨の萎縮などを含む種々の理由から困難な手法であり、さらに上顎洞底挙上術を必要とする症例が多い。上顎洞底挙上術が失敗すると上顎洞炎および炎症による骨破壊を招く合併症をもたらす可能性がある。両合併症はしばしば患者に後遺症をもたらすことから、上顎洞底挙上術に対するガイドラインを検討することが重要である。
 近年、上顎洞底挙上術においては、ラテラルアプローチとインターナルアプローチの2つが用いられているが、両方法が有する利点と欠点により、異なる状況でのガイドラインに関して、種々の相違意見が存在する。
 今回の講演では、上顎臼歯部におけるインプラント埋入を行う際に考慮すべき事項、ならびに上顎洞底挙上術を安全に行うために私が行っている取組みについて話をする予定である。

Using 3D Radiography in Clinical Orthodontics

The digital volumetric tomography era has begun, and we now have access to significant additional diagnostic information. This presentation will show how Case Western Reserve University (CWRU) is extracting and using volumetric tomograms on the day to day basis for every orthodontic patient in our clinic. From posterior buccolingual inclination to surface area analysis and volumes, Dr. Palomo will show current CWRU's protocols and recommendation on what to do with this 3D imaging modality that is quickly becoming part of the routine records.

矯正臨床における3次元エックス線画像の使用法
 デジタル容積断層画像の時代が始まり、今や診断面でもきわめて多くの情報を得ることが可能になってきた。この講演では、ケース・ウェスターン・リザーブ大学(CWRU)の日常臨床で行っている矯正患者の容積断層画像の抽出法とその使い方について説明し、とくに臼歯部の頬舌傾斜計測から顔面表面の分析、そして容積解析など、急速に日常の診断記録になりつつある3次元画像の推奨できる事柄と、CWRUの最新計画などを講演する。

Periodontics-Orthodontics: CBCT enhances multidisciplinary outcomes

The classic meeting-point between periodontics and orthodontics is in the management of patients with plaque-induced inflammation. Today this relationship is broadened to include periodontal-plastics, reconstructive, and dental implant procedures all with a goal of enhancing overall orthodontic outcomes. The advantages of three-dimensional imaging offer the most exciting cross section of perio-ortho multidisciplinary cases. This presentation focuses on our most interesting cases, and highlights CBCT usefulness in enhancing treatment outcomes. Cases shown will include trauma, exposure of impacted teeth, dental implants, skeletal anchorage (TAD’s), and Case’s protocols for more predictable and reliable outcomes.

歯周病学と歯科矯正学:CBCTがマルチディシプリナリー臨床にもたらすさまざまな治療成果
 歯周病学と歯科矯正学の間で行われてきた古典的なポイントは、プラークがもたらす炎症患者の管理のなかに存在する。今日、この両者の関係は矯正治療全体のゴールに関連する歯周組織の再形成、再構築そして歯科用インプラントなど、これらすべてを含む手法へと広がりをみせている。3次元画像は歯周―矯正のマルチディシプリナリーな症例に対し、非常に興奮させる面を提供してくれる利点がある。今回の講演ではもっとも興味深い症例と、治療成果を高めようとする際に必要な話題のCBCTの使い方などに焦点を当ててお話しする。提示する症例としては、外傷、埋伏歯の開窓、歯科インプラント、骨固定(暫間的固定装置)、そして、より予知性が高く信頼性のある治療結果を得るための治療計画について講演する。

Direct Esthetic Restorations: a rational approach.

In the last ten years composite materials made dentistry more conservative.
Nowadays borders between operative and prosthetic dentistry are very difficult to point out.
Nevertheless techniques become more difficult to realize with a complex treatment plane and new concepts to learn.
The reliability of adesive techniques and the increased aesthetic requirements of the patients impose the review of the classics concepts about the restorative dentistry.
The main of the lecture is to illustrate the characteristics of the materials, the possibilities of use, and the clinical sequences of the most recent techniques for the conservative restorations with direct technique.

直接法審美修復;その合理的アプローチ
 過去10年の間にコンポジットレジンによって歯科はより保存的になった。 今日では保存学と補綴学の境界は判らなくなっている。 しかし 複雑な治療計画を行うための手技や習得すべき新概念もより難しくなっている。 接着の信頼性と審美性の要求のたかまりによって、保存修復学の旧来の概念を見直さなければならなくなった。 本講演では、主に材料の特性とその可能性を明らかにし、直説法の保存修復の最新のテクニックの臨床でのながれを明示する。

Dr. Ariel J. Raigrodski

 

Dr. Raigrodski is an Associate Professor and Director of Graduate Prosthodontics at the Department of Restorative Dentistry at the University of Washington. He is a member of the editorial review boards of Practical Procedures and Aesthetic Dentistry, the Journal of Esthetic and Restorative Dentistry, the Journal of Prosthodontics, and the Journal of Prosthetic Dentistry. Dr. Raigrodski is a graduate of the Hebrew University in Jerusalem Israel. He received his certificate in Prosthodontics at Louisiana State University School of Dentistry, where he also completed a fellowship in implants and esthetics, and an MS degree. He is a Diplomate of the American Board of Prosthodontics, a fellow of the American College of Prosthodontists, A fellow of the International College of Dentists, a member of the American Academy of Fixed Prosthodontics, the Academy of Osseointegration and other professional organizations. A co-editor of the English version of the book “All-Ceramic At A Glance”, Dr. Raigrodski’s research focuses in all-ceramics and CAD/CAM technology. He lectures both nationally and internationally, and holds a private practice in Kenmore, WA.

Zirconia-Based Restorations-A Comprehensive Approach

This presentation will discuss the mechanical as well as optical properties of zirconia and the indications and limitations of zirconia-based restorations. The program will address the question of how the advent of CAD/CAM technology as well as veneering porcelain application techniques, support the fabrication of these restorations including the use of zirconia abutments. Comprehensive cases will be presented to demonstrate the use of zirconia-based restorations as part of an interdisciplinary treatment approach.

Dr. Andrea Ricci

 

Dr. Andrea Ricci graduates from University of Perugia in 1996. After a three years training program he achieved the Certificate in Advanced Prosthodontics at the University of Southern California in Los Angeles. Since 1999 he limits his practice to prosthodontics, periodontology and implant dentistry with special emphasis to the aesthetic aspects. He is an Active Member of the European Academy of Esthetic Dentistry (EAED). Co-author of several papers published in international journals, he has lectured extensively in Europe and in the United States.

Esthetic solutions to restore patients with altered hard and soft tissues profiles

Advanced periodontial disease, consequences of periodontal therapy, iatrogenic lesions, traumas may lead to different esthetic problems such as elongated teeth, altered gingival margins, migration of teeth, lack of three-dimensional hard and soft tissue volume and flattening hard and soft tissue in extraction sites.
The presentation will focus on the solutions of these problems, on an adequate diagnosis of the different sites, on the forecast of the clinical outcome and on the technical solutions, which, in most cases, requires a multidisciplinary approach which involves the effort of the entire dental team.

硬組織と軟組織の造成を伴った審美修復
 重度の歯周疾患、歯周治療後の(思わしくない)結果、根尖病巣、外傷などは、修復歯冠を長くしたり、マージンの位置変化、歯の位置変化、三次元的硬組織、軟組織の欠損、抜歯部位の硬組織、軟組織の平坦化というさまざまな審美的問題の原因となりうる。
 本講演では、これらの問題に対する対処法、症例に対する適切な診断、臨床結果の予測、そして技術的手法に焦点をあてる。ほとんどの症例においてマルチディシプリナリーアプローチが必要とされ、歯科チーム全体の協力が重要となる。

Dr. Avishai Sadan

 

Dr. Avishai Sadan is Dean and the G. Donald and Marian James Montgomery Professor of Dentistry at the Herman Ostrow School of Dentistry of USC, University of Southern California (USC) in Los Angeles, CA.
He is the former editor-in-chief of Quintessence International, the former editor-in-chief of Quintessence of Dental Technology (QDT), served on the editorial boards of other journals, and has lectured and published nationally and internationally on esthetic and implant dentistry and biomaterials. Dr. Sadan is a graduate of Hebrew University Hadassah School of Dental Medicine in Jerusalem, Israel where he has also completed an internship in maxillofacial prosthodontics and oral medicine. He received his training in prosthodontics from the LSU School of Dentistry and has also completed a fellowship in prosthodontics emphasizing esthetic and implant dentistry at LSU. A member of several professional organizations including the American College of Prosthodontists, the Academy of Osseointegration, the American College of Dentists, OKU Dental Honor Society, Pierre Fauchard Academy, and other organizations, he maintains an intramural practice limited to fixed and implant prosthodontics.

Stem Cells for Grafting of the maxilla in combination with dental implants

Disadvantages of autologous bone for augmentations are the limited availability and the necessity of an additional surgical procedure.
Ideally, less invasive regenerative procedures are used under outpatient conditions, using only autologous material with boneforming ability.
In a RCT 45 sinus were augmented with BioOss and stem cells (test) and 25 sinus were treated with a mixture of BioOss and bone (control). Biopsies were obtained 3 months later. New bone formation was comparable between both groups with less donor site morbididty in the test group.
Further investigations are necessary to determine the possible future role of stem cells in augmentation procedures.

上顎のインプラント埋入と併用して行う幹細胞移植術
 骨造成に自家骨を利用する際の欠点は、採取量の制限、ならびに追加外科手術の必要性であるが、骨形成能を有する自己由来材料のみを利用し、外来通院下で行う低侵襲な再生療法が理想的である。
 今回われわれは、無作為化比較対照試験を行った。実験群の上顎洞(n=45)にはBio-Ossと幹細胞を移植し、対照群の上顎洞(n=25)にはBio-Ossと骨の混合物の移植をした。その3ヵ月後に生検を行った結果、新生骨の形成が両群で同程度に認められ、実験群では供給側における合併症がほとんど認められないことが明らかとなった。
 造成術において、今後起こりうる幹細胞の役割を決定付けるにはさらなる検索が必要であろう。

Mr. Stefan Schunke

 

I was born in 1958 in Leverkusen/Germany.
In 1976, I started an apprenticeship as a Dental Technician in Leverkusen/Germany. After graduation in 1980, I attended a course in waxing up and Gnathologie. Since then I have been highly interested in the occlusal surfaces of the fully operational, esthetical and healthy set of teeth and wanted to learn about the fundamental. This is why I started to work for Mdt. Bölte in Düsseldorf/Germany, worked for them for three years and got my degree as a supervisor externally. Several courses followed that added to my knowledge. Through theses classes I recognized the value of the biomechanical waxing up from M. H. Polz. In 1988, I started working for the "Zahntechnisches Laboratorium M.H. Polz" and became partner. Since 1991, M. Polz jr. and I are partners and owners of this laboratory.
I worked for the "Bensheimer Arbeitskreis" for several years.
Other courses, seminars, conferences and classes followed, given by teachers such as: M.H. Polz, P.K. Thomas, A. Lauritzen, V. Bauer, A. Gutowski, D. Kubein-Meesenburg, C. Lex, W. Schöttl, R. The human and his dentures - or - the dentures and his human?

This lecture is under the main topic of occlusion. Now we can talk about the occlusal stops and the different movements. The single teeth of their own, the morphology, the stops and the knowledge of the movements are one point. But this is only a small part of the world called "patient or human". But with the treatment the dentist and the work of the technician influences much more.
In this lecture will be discussed the relationship between posture-muscle-teeth and of course occlusion and movements. How can we correct managing or placing the teeth in a three dimensionally area? With the according background knowledge a correct Model analysis can help the dentist to decide the best treatment plan. Because what happens during lifetime the patient we can find on the teeth and especially on the model. If this is true we must find the same in the face and on the posture of the patient. With this it will be carve out the relationship between function and esthetic. We will talk about different plans and the difficulties and what plan we can use.
All this will be shown with cases.

人のための義歯なのか? 義歯のための人なのか?
 本講演では、姿勢・筋肉・歯の相互関係、また、咬合と機能運動について考察する。どのようにすれば、歯を3次元的な位置に正しく配置することが出来るか? 基本的な知識があれば、作業模型を分析することによって、歯科医師は最善の治療方針を計画することが可能になる。また、作業模型を観察することで患者の生活習慣が推察できるように、表情や姿勢からも患者の生活習慣に関する情報を得ることができる。これらをもとに、機能と審美の関係を考察することも可能なのだ。単冠歯の補綴においても、形態・咬合・機能運動など、考慮するべき重要な要素は多い。しかし、患者を1人の人間としてみたとき、これらは重要な要素の中の1つに過ぎないと分かるだろう。患者の治療においては、歯科医師と歯科技工士の及ぼす影響は計り知れない。
 以上を踏まえたうえで、実際の臨床例を通して、さまざまな技巧のテクニックをその難易度とともに考察していく。

Dr. Tota Shimizu

 

Dr. Shimizu is a Clinical Assistant Professor at USC School of Dentistry, and maintains a private practice limited to endodontics in Glendale, California. He received his DDS degree from Kagoshima Univ, Japan in 1993, and completed his fixed prosthodontics training by Dr. Sumiya Hobo in 1995. In 2000, he obtained his specialty certificate in Advanced Endodontics from USC School of Dentistry. He lectures nationally and internationally on microscopic surgical endodontics.

外科的歯内療法の現状と今後の展望

 マイクロスコープやNi-Tiファイルなどの技術革新により,歯内療法が高い予知性をもって行えるようになった一方,外科的歯内療法の重要性はかえって,いや増すばかりである.90年代中盤にゲイリー・カーらにより提唱された新しい外科的歯内療法のプロトコールがいまや米国歯内療法のなかで確固とした地位を占めていることがその何よりの証左であるが,その一方で2010年現在,その再考を強いる臨床的エビデンスが続々と明らかになりつつあるのもまた事実である.
 本講演においては外科的歯内療法のこれまでと現状,そして今後の新しい方向性について,米国臨床エンドドンティクスの現場から報告を試みてみたい.

Mr. Hans-Peter Spielmann

 

Hans-Peter Spielmann, born in Switzerland, graduated in Dental Technology in 1968. He received his Master of Science Degree at Boston University of Graduate Dentistry in 1981. From 1973 to 1981 he gained additional professional experience in several places abroad, such as Cape Town, Rio de Janeiro, Rome and Boston. Back in Switzerland1981, he started his own laboratory. He lectured in his home country as well as abroad on all subjects of Dental Technology. He also published numerous papers on ceramo-alloy, implantology and CAD/CAM technology.

Prosthetic and Laboratory Considerations in Single Tooth Restorations in the Esthetic Zone - Predictability Through Connectional Approaches

Proper diagnosis and planning of implant placement and the restoration are prerequisites for predictable, functional and esthetic restorations. Implant placement must be as optimal as possible in saggital as well as vertical position according to the size of the tooth to be restored. This is especially true in the anterior region, where esthetics becomes the utmost challenge to the dental treatment team. The strict observance of the treatment and laboratory protocols enables the treatment team to maintain a high level of reproducibility. Cooperation among the treatment team: surgeon, restorative dentist and technician is mandatory to achieve a predictable, functional as well as an esthetic performance of long lasting restorations that blend in perfectly and emerge from the peri-implant sulcus in harmony with neighbouring teeth.
We firmly believe that only the compliance with a tight treatment concept together with perfect team-work as well as some experiences will lead to the desired results. It is the aim of this lecture to show various solutions of superstructures and laboratory procedures for anterior restorations with the importance of the above mentioned cooperation to finally achieve optimal results.

審美領域における単独歯補綴物に対する補綴ならびに技工的な配慮
―連携アプローチによる予知性の向上―

 インプラント埋入と補綴物に関する適切な診断・設計は、予知性が高く機能的・審美的な補綴物のための必須条件である。インプラントの埋入位置は、矢状・垂直いずれの方向においても補綴される歯の大きさに見合った最適なものでなければならない。これはとくに前歯部に当てはまるが、この部位は歯科治療チームにとって審美性が最大の課題となる。治療チームは治療・技工の手順を厳格に遵守することによって、再現性の高いレベルを維持することができる。予知性が高く機能性にすぐれ審美的な結果が長期にわたって維持できるような補綴物を製作するには、治療チーム内(口腔外科医・補綴医・歯科技工士)の協同関係が必須である。そのようにして作られた補綴物は周囲に完全に溶け込み、隣接した歯と調和し、インプラント周囲の歯肉溝から現れる。

Dr Roberto Spreafico obtained his DM degree at Turin University, Italy, in 1982. In 1986, he obtained a DMD degree, at Geneva University, Switzerland. He is now a private practitioner, in Busto-Arsizio, near Milan, Italy. He is active member of: Accademia Italiana di Conservativa, European Academy of Esthetic Dentistry, Associate Editor of “European Journal of Esthetic Dentistry”. Member of the editorial board of: Journal of Adhesive Dentistry; Practical Procedures & Aesthetic Dentistry. Dr Spreafico presently conducts a number of courses in the field of esthetic dentistry throughout Italy and abroad. He is also author of numerous clinical papers on this topic. ANTERIOR COMPOSITE RESTORATIONS: ACHIEVING NATURAL ESTHETICS WITH A CONSERVATIVE TREATMENT

Today's dentist has a wide choice of restorative materials available when replacing missing or damaged tooth structures in both anterior and posterior teeth.
Anterior restorations can be made with ceramic or composite. With ceramic we can do just indirect restorations like full crowns or veneers. The indirect technique however requires a dental preparation with a certain sacrifice of sound tissues.
Direct composite restorations, at the contrary, allow a more conservative treatment with a minimal dental substrate removal.
Since introduction in dentistry of composite resins, those materials have undergone a tremendous development and their adhesive properties on both enamel and dentin have been constantly improved, allowing a more conservative approach during the tooth preparation. Patients’ demands for a more aesthetic and metal-free restoration on posterior and anterior teeth increased during the last decade focusing the interest of the dental profession towards aesthetic restorations with composite resins.
However, it is vital that the treating clinician have a thorough understanding of the principles that lead to a successful outcome.
Naturally appearing and problem free outcome can be achieved if certain rules are followed by a conscientious operator.

前歯部のコンポジットレジン充塡:修復治療における審美を達成するために
 前歯および臼歯部を修復する場合、今日では幅広い選択肢がある。前歯部の修復はセラミックス、あるいはコンポジットレジンで行われる。セラミックスにおいては、フルクラウンやベニアのような間接修復のみが可能で、これは健全な歯質をある程度犠牲とする形成を必要とする。
 これに対して、直接法のコンポジットレジン修復では最小限の歯質削除による、より保存的な治療が可能となる。コンポジットレジンが登場してから、材料は凄まじい進歩を遂げ、エナメル質と象牙質への接着特性も改善されている。これにより、形成時により保存的なアプローチが可能となってきた。患者の要求は、前歯部と臼歯部においてもより審美的に、またメタルフリーの治療となり、ここ十年程の間にコンポッジットレジンによる審美的修復への関心が高まっている。
 しかしながら、治療を成功へと導くための原理・原則を臨床医がしっかりと理解しておくことは必須である。一定のルールを注意深く守れば、自然な外観とトラブルのない結果をだすことが可能となる。

Dr. Henry H. Takei

 

Dr. Henry H. Takei is a Professor of Periodontics at the University of California, Los Angeles (UCLA) and has previously served as Director of the Post Doctoral Program in Periodontics.
Dr. Takei is also a Consultant in Periodontics to the Veterans Administration Hospital in Los Angeles.
Along with his educational activities at the university, he also maintains a private practice Limited to Periodontics and Implant Surgery.
Dr. Takei has presented more than 600 lectures and seminars in the United States, Canada, Europe, Asia and South America. He has also given over 200 Continuing Education Courses for UCLA. He has published numerous articles in scientific and professional journals and contributed to five textbooks on periodontics and implant surgery. Dr. Takei is currently Co-editor of the classic textbook, “Carranza’s Clinical Periodontology”, now in its 10th Edition. He founded the first formal UCLA Continuing Education Courses with universities in Japan, as well as the first undergraduate Student Exchange Program, also with Japan. This program has been permanently endowed. He has been appointed as a Visiting Professor at several dental schools in Asia.

As a faculty member at UCLA, he has been selected as the Outstanding Professor by eight graduating classes and received the Alpha Omega Educator Award in 1975. In 1998, the UCLA Alumni Association recognized Dr. Takei as the Honorary Alumnus of the Year. A scholarship has been established in his honor for his service and dedication to students. He was also honored by Marquette University in 2001 as their “Distinguished Alumnus of the Year”. Recently, the American Academy of Periodontology honored Dr. Takei with the “Master Clinician Award, 2006” to recognize his clinical contributions to the profession. This award is the highest clinical recognition by this academy.
Dr. Takei holds membership in numerous professional organizations and is a Fellow of both the American and International Colleges of Dentists. He is also a member of Omicron Kappa Upsilon, American Dental Association, American Dental Education Association, American Academy of Esthetic Dentistry, Pierre Fauchard Academy and the American Academy of Periodontology. He has been selected for honorary memberships in numerous periodontal societies internationally.

SAVING THE NATURAL DENTITION OR EXTRACTION AND IMPLANT THERAPY

The evolution of dental therapy is currently undergoing a major "paradigm shift" due to the advent of predictable implant therapy. The factors influencing this shift affects every discipline of dentistry from periodontics, oral surgery, endodontics, orthodontics to restorative dentistry. The inclusion of implant therapy has altered the criteria for treatment planning of retention or extraction of a diseased dentition. Understanding and communicating the various options and their ramifications becomes more critical in patient education and case management. There are numerous factors to be considered in this important decision making process which will be addressed in this presentation.

Immediate loading : what applications can we expect in the future?

The widespread therapeutic use of dental therapeutic alternative under certain implants over the last 20 years has led to the revision of several aspects of the original two-stage Brånemark protocol, developed early in the 1970s. After using the single-stage approach as a valid treatment procedure for many years, one of the most dramatic changes in implant dentistry has been the increased acceptance of immediate loading protocols as a viable therapeutic under certain circumstances. The ultimate goal of an immediate loading protocol is to reduce the number of surgical interventions and shorten the time frame between surgery and prosthetic delivery, all without sacrificing implant success rates. These new protocols will ultimately lessen patient’s reservations and result in increased acceptance of implant therapy. Before embracing the procedure as a routine treatment, the immediate loading technique needs to be validated with a significant number of clinical cases, extended follow-ups as a clear definition of limitations. Immediate occlusal loading procedures can be successful only when the amount of micro-motion at the bone-implant interface is kept beneath a certain threshold during the healing phase. This procedure, although predictable, is technique-sensitive and should be applied cautiously. A gradual and progressive approach to immediate loading is therefore recommended.
The program will address a variety of topics related to immediate loading in different clinical situations: edentulous mandible and maxilla, partially edentulous patients from single tooth to multiple unit. A review of the available literature will be presented along with the biologic rationale for Immediate Loading. The case selection process and step by step surgical prosthetic procedure to achieve a predictable result will be discussed.

即時荷重:将来にわれわれが期待できる適応症とは?
 過去20年間にわたる歯科治療の選択肢としてインプラントを利用することにより、1970年代初頭に提唱されたオリジナルである2回法のブローネマルクプロトコールが見直されてきている。長年、有効な治療術式として1回法が用いられ、ある状況下では、即時荷重プロトコールが実行可能な治療法と考えられており、このことは、インプラント歯学におけるもっとも劇的な変化のひとつである。即時荷重プロトコールの最終目標は、インプラントの成功率を変化させることなく外科介入の回数を減少させ、外科から補綴へ移行する時期を短縮化することである。最終的にはこれらの新しいプロトコールにより、予約回数の減少とインプラント治療における適応拡大をもたらすであろう。通法として容認される前には、数多くの臨床例、ならびに長期フォローアップのような限定された明確な定義を用いて即時荷重テクニックの正当性を立証する必要がある。即時機能荷重は、骨-インプラントにおける微小動揺が治癒期間中に一定の閾値以下であり続けた際に初めて奏功する術式である。この術式は予知性が高いがテクニックセンシティブであることから慎重に行われるべきで、即時荷重に対する漸進的なアプローチが推奨される。
今回われわれは、無歯顎(上下)患者、ならびに1歯欠損から多数歯欠損までの患者といった異なった臨床的な状況での即時荷重に関連した種々のテーマについて講演を行う。利用可能な文献のレビューにより即時荷重の生物学的理論が見出されるであろう。予知性の高い結果を得るための患者の選択過程、ならびに着実な外科補綴術式についても検討するつもりである。

Dr. Hom-Lay Wang

 

Dr. Hom-Lay Wang, Endowed Collegiate Professor of Periodontics, Professor and Director of Graduate Periodontics at the University of Michigan, School of Dentistry. Dr. Wang received his DDS from Taipei Medical College, Taipei, Taiwan, DDS, MSD from Case Western Reserve University, Cleveland, Ohio and Ph D from Hiroshima University, Japan. In 2006, he was awarded an honorary Ph D from University of Medicine, ViCTOR BABES, Timisoara, Romania to honor his contribution in Dentistry. He is the author or co-author of 20 book chapters/invited reviews and more than 180 scientific articles. Dr. Wang is a Director and Diplomate of the American Board of Periodntology (ABP), a former President of the Midwest Society of Periodontology, a former Consultant for the ADA Commission on Dental Accreditation, and a President-Elect, Board Director of the International Congress of Oral Implantologists (ICOI) and a Fellow of American College of Dentists.

He serves as an Editor-in-Chief for the Open Dentistry Journal, an Associate Editor for The International Journal of Oral & Maxillofacial Implant, International Chinese Journal of Dentistry and Assistant Editor for the Implant Dentistry, Editorial Board member for the Journal of Periodontology, Clinical Oral Implants Research, International Journal of Peridodontics & Restorative Dentistry, Journal of Clinical Periodontology, European Journal of Dentistry and many others. Dr. Wang is the recipient of following awards/honors: AADS clinical research fellowship award (1992), Best Faculty Award (2003), The Charles E. Immediate implant placement with or without bone grafting: Flap vs. Flapless

Immediate implant placement (IIP) was proposed to shorten treatment time and to promote implant esthetic. The purposes of this presentation are to 1) review and evaluate literature in the field of IIP, 2) discuss factors that may influence this treatment modality, 3) discuss when to place bone graft and when not, 4) address when to use flap or flapless approach, 5) propose treatment decision tree for selecting either immediate or delayed implant placement, 6) determine when the IIP can be preformed in the anterior esthetic area and 7) demonstrate cases that were successfully treated with IIP. The findings from our recent research will be presented and discussed. In addition, the advantages and limitations associated immediate implant placement will be analyzed.

骨移植の有無によるインプラントの即時埋入(フラップvsフラップレス)
 インプラントの即時埋入(以下IIP)は治療期間を短縮し、インプラント審美を促進するために提唱された。今回の講演における目的は、1)IIPの分野に関する文献をレビューして評価すること、2)IPPの治療方法に影響を与える要因について検討すること、3)骨移植の有無について検討すること、4)フラップもしくはフラップレスアプローチを検討すること、5)インプラントの埋入方法(即時もしくは遅延)を選択するための治療のフローチャートを提案すること、6)前歯部審美領域においてIIPが可能な時期を決定すること、ならびに、7)IIPにより成功した症例を提示することである。われわれの近年の研究における知見も交えて検討する予定であり、さらにはインプラントの即時埋入に関連する利点および限界に関しても細かく検討する予定である。

Mr. Aki Yoshida

 

Aki Yoshida started his career at the Tanaka Dental Clinic in Tokyo, Japan after graduating from the Dental Technician School of Nihon University in 1983.
In 1991, Aki came to the United States to accept a position at Gnathos Dental Laboratory, which the renowned prosthodontist, Lloyd L. Miller, D.M.D.
In addition he serves as a technical instructor in the Department of Post-Graduate Prosthodontics at Tufts University in Boston, an adjunct faculty member of the UCLA Center for Esthetic Dental Design and an instructor for Noritake Dental Supply USA, an associate member of the American Academy of Esthetic Dentistry. Aki Yoshida has presented numerous hands-on courses and lectures throughout the United States, South America and Europe. He is a four-time masterpiece Technician in the Japanese Quintessence of Dental Technology, and he has published several articles on anterior esthetic restoration in the US version of QDT. His most recent accomplishment, owning the very lab he first began in. Currently it is known as Aki Yoshida, Inc DBA Gnathos Dental Studio.

Zirconia Restorations; Color Evaluation by Spectrophotometer and Porcelain Build-up Method

We have been told that Zirconia coping has a strong opacity, but in clinical cases we can see that in strongly discolored case, the tone of the color can permeate through the coping. In a strongly discolored case like this, it is necessary to add the right amount of opacity to the coping in order to create the esthetical color build up. Therefore, it is important to understand the final restoration and the die color accurately, and by analyzing the color in numbers made possible to create the chart of the recommended masking material. Also, by using the spectrum photometer to analyze the shade color in three dimensions, we are able to choose the right porcelain powder to minimize the remake of restoration, and also examine the porcelain build up technique will be done.


ジルコニアレストレーション;分光光度計による色調の評価と陶材築盛法

 ジルコニアによるセラミックコーピングは非常に不透明度が高いと言われてきたが、強度の変色支台歯の場合、その色調が透過してしまうことが臨床例で確認できる。 このような重度の変色を伴う支台歯の条件下においても、審美性に優れた色調構築を可能とするためには、 ケースに応じた適切な不透明度をジルコニアコーピングに付与することが必要となる。適切な不透明度を得るため、最終補綴物と支台歯との色調差を正確に把握する必要があり、色調差を数値として表すことにより推奨するマスキング材をチャートにすることが可能となった。
 またスペクトロフォトメーターを用いたシェードテイキングと3次元的な色調分析により、再製を最小限にするためのポーセレンパウダーの選択、築製法についても考察していきたい。

Dr. Orcan Yüksel

 

Dr. Orcan Yüksel visited the Johann Wolfgang Goethe University in Frankfurt, Germany and the University of Istanbul, Turkey and graduated in 1987. He pursued his Doctorate degree at the Frankfurt University.
He is a member of several German Dental Associations and a certified Implantologist in Germany /Europe and Diplomate of ICOI in USA and also the EDAD (Turkish Academy of Esthetic Dentistry) and representative of this academy in Germany. He is also one of the editors of the Quintessence magazine in Turkey. He is founding member of Omega Group and Honory Member of some Implant Societies.
Dr.Yüksel is also a certified instructor on Implantologie. He and his Team give monthly periodic lectures to his colleagues worldwide on Implantology and also he is teaching in Implantologie Curriculum Program with the University in Frankfurt.
He has been practicing in Frankfurt Germany since 1993 in his own clinic, specialized on Esthetic and Implant Dentistry.

Aesthetic with implants is not a Coinsidence

The proper localisation of implants plays a major role in immediate and late Implant Placement for a Successful Treatment.
When the anterior teeth are afflicted with the loss of soft and hard tissue after naturally healed extraction socets, the prosthetic of the patient from the aesthetical point of view leads mostly to dissatisfactory results. What are the reasons for good results? Under which conditions can a good result be expected.
Should the loss on hard tissue be regenerated via the "guided bone regeneration" method in order to achieve a better level of red aesthetics? These methods must be predictable. This presentation covers latest techniques in the field of Implants and Aesthetics. A convincing treatment concept for extraction sockets (socket preservation), in which implant placement can achieve good results after several months will be presented. The technique of Bone Rings: "3-Dimensional Vertical Bone Reconstruction in One Step Procedure" is very promissing. The augmentation of bone defects and the insertion of dental implants in a single surgical procedure with using an Cortico-Cancellous Autogeneus Grafts from the chin can reduce the treatment time and achieve aesthetical excellent results.

インプラント治療における審美の必然性
 結果を成功に導くためには、即時あるいは待時埋入どちらもインプラントの適切な埋入位置がとても重要となる。前歯部において抜歯治癒後の硬組織、軟組織の欠損症例では、患者の補綴物に対する評価は、ほとんどの場合満足できないものとなる。良い結果を得るためには何が必要か? どのような状態であれば良い結果が得られるか? 歯肉のレベルを良い状況にするために、GBR で骨を再生するべきか? これらの方法は予知性の高い方法でなければならない。本講演では、審美性の高いインプラント治療の最新のテクニックについて発表を行う。
 まず、抜歯窩に対して信頼性の高い方法(ソケットプリザベーション)を紹介する。本法では、数か月後にインプラントを適切な位置に埋入することができる。外科処置が1回のBone Rings法(垂直的三次元骨増生)は、とても有効である。骨の欠損部に患者自身のオトガイから採取された皮質骨、海綿骨を使用する骨増生法と、インプラント埋入を一度の外科処置で行う手技は、治療時間の短縮とかなり審美性の高い結果を達成することができる。

The evolution of dental ceramic on implant and natural teeth related to dental aesthetic results.

The use of CAD-Cam techniques in dental field has revolutionized the dental techinician practice.
Systematic use of information technology in the lab together with traditional handicraft demands us to study to discover new prospects.
The actual change is also underlined by new materials,such as zirconia for its aesthetic outcomes and bio-functional results.
Companies put a lot of effort to improve quality relating to dental ceramic and we expect they will improve even more.
Zirconia is an innovating product that gives interesting prospectives in the traditional ceramic topic but its recent attainment does not give us the chance to deeply know its full potential, differently from allumina which I think has more pros at the moment.

天然歯とインプラントの審美修復における歯科用セラミックスの革新
 今日、歯科技工所におけるCAD/CAMの使用が飛躍的に増加している。その新しい情報や技術を従来の手作業に融合するために、われわれは新たな試みや研究をする必要がある。
 その中でも、変革のひとつとして挙げられるのが、審美的修復や生態的に良好な結果を出している材料であるジルコニアの存在だろう。現在、企業は歯科における各種セラミックスの向上に注力しているが、われわれはさらなる向上を求めている。
 ジルコニアは革新的な材料であり、従来のセラミックからみても非常に面白い見込みのある材料だと考えているが、現状の学識では考えられる可能性やアルミナとの違いも十分に理解されていないようにも思える。
